On Task Behavior 
(being in the appropriate place, working on the given task, not chatting during work time, not doodling or writing notes during class, etc.) 
3pts- The student is always on task. 
2pts- The student is off task sometimes during the school day. 
1pt-   The student is off task often during the school day.  

Inappropriate Behaviors (Shouting out, throwing things, horse playing, being disrespectful, etc.) 
3pts- The student does not exhibit inappropriate behaviors during the school day.  
2pts- The student sometimes exhibits inappropriate behaviors. 
1pt-   The student exhibits inappropriate behaviors on a regular basis. 

Following Routines  (Following the routines for the hallway, morning work, dismissal, drills, lunch, bathroom, turning in work, etc.) 
3pts- Follows all classroom routines on a consistent basis. 
2pts- Teacher has to remind the student of the classroom routines.  
1pt-   Student disregards the classroom routines on a regular basis.  

Office Referrals (extreme behavior or consistent misbehavior will be referred to the office) 
3pts- Student was not referred to the office at all during the current 9 weeks. 
2pts- Student was referred to the office once during the nine weeks.  
1pt-   Student was referred to the office more than once during the current nine weeks. 
  
11 or 12 points= Satisfactory 
9 or 10 points=Needs Improvement 
8 or less points= Unsatisfactory
